Library Cards
Library cards are issued to all Evergreen Park residents without
charge upon proof of residency:
- A current photo ID.
- One other current form of identification
with preprinted name and Evergreen Park address.
If the address on the photo ID is not
current, applicants must produce two current forms of identification.
There is no minimum age to apply for a card, but a parent or legal
guardian must present proof of Evergreen Park residency and sign
the application in person for a child under age 18. Library cards
are valid for three years, and can be renewed at no charge.
Proof of Residency
To apply for a card, bring in a photo
ID with your current Evergreen Park address and one other current
form of identification preprinted with your name and Evergreen Park
address such as lease or mortgage papers; utility bill; motor vehicle
registration or voter's registration card. There is no minimum age
to apply for a card, but a parent or legal guardian must present
proof of residency and sign the application of a child age 18 or
under.
Lost or Stolen Cards
For the owner's protection, lost or stolen library cards should
be reported to library staff immediately. The owner is responsible
for all materials checked out on the card until the library is notified.
A lost or stolen card can be replaced for a $2.00 fee.
Non-Resident Library Cards
Individuals residing outside of Evergreen Park whose closest public
library is the Evergreen Park Public Library, and who do not reside
within the boundaries of another public library, and who do not
own taxable property within Evergreen Park, may purchase a non-resident
library card for a fee.
Business Library Cards
Individuals residing outside of Evergreen Park who own taxable property
as an individual, partner, principal stockholder, or other joint
owner within the Village of Evergreen Park may be issued one non-resident
library card without payment of the non-resident fee. The Senior
Administrative Officer of a firm, business, or other corporation
owning taxable property in Evergreen Park may be issued one non-resident
card without payment of the non-resident fee. An officer of the
business must present the most recent tax bill upon the taxable
property. Use is limited to the individual whose name appears on
the card. A business card is valid for one year, and can be renewed.
Reciprocal Borrowers
If you have a library card from one of
the Metropolitan Library System libraries, you may use that card here.
Chicago Public Library patrons may apply for a Reciprocal Borrowers
card with a valid Chicago Public Library card and two current forms
of identification. There will be a wait of about two weeks to
receive the card. The Reciprocal Borrowers card is good for one
year and can be renewed. If you have a library card from one of
the other systems in Illinois, you may also use that card here.
